Step 3

Targeted File Review & Coding (Programme Scope)

Purpose:
To convert "possible" candidates into a confirmed, usable programme cohort by reviewing only the Patients identified in Step 2.

When this is appropriate:

Best suited to practices where Step 1 indicates that coding and structured data have been used reasonably consistently over time 

What is done: 

  • Confirm true eligibility for CDM, PP and OCF on the Step 2 candidate lists

  • Apply programme-scope coding only (the minimum required for reliable delivery, recall and reporting)

  • Produce clean, confirmed cohorts ready for registration, recall and scheduling

Limitations:

  • Any Patients not identified in Step 2 will not be reviewed here

  • This is a cost-contained improvement, not a full data base correction

  • Does not correct deeper historical data gaps

Outcome:

  • Confirmed, reliable programme cohorts

  • Improved programme-relevant coding and reporting accuracy
