Baseline & Benchmark (Reality Check)

Purpose:

Establish a clear, objective starting point and quantify likely leakage so progress can be measured.

What is done:
• 12‑month lookback using the EMR
• Review of current CDM / Prevention / OCF activity as recorded and submitted
• High‑level comparison to theoretical potential based on national averages
• Identification of operational blockers and data quality issues
• Identification of whether the database appears broadly representative or materially incomplete 

Output:
• A 1‑page GP summary covering:
– Where the practice appears to be now
– Where the main bottlenecks are
– An indicative estimate of unrealised potential (with clear caveats)
– A recommendation on the most appropriate next step (Targeted route vs Deep Dive route) 

Key principle:
This step is a reality check, not a judgement. It sets direction.
